Procedures taken during the death investigation of Naval personnel can vary on a case-by-case basis. In most instances, interviews and interrogations take place after a report has been filed, or upon the opening of a case, however, in some instances it is harder to find witnesses or suspects, and agents must rely on evidence collected and gathered. Each case has an individual file identified by a specific number that contains all procedures taken. Files can include notes from interviews and interrogations, photos, phone records, technological conversations including text messages, emails, or instant messenger conversations, DVDs or CDs showing evidence such as surveillance tapes or files from a computer and the summary of the case including the specific details available. Often times, when a man, woman, or child passes away without any signs of illness or otherwise noted pertinent medical history, an autopsy is performed. Likewise, in cases involving homicide, autopsies are also performed. An autopsy is a medical examination that occurs post-mortem to determine the cause of death and the manner of death. Autopsies are also able to determine if the deceased had any other medical diagnoses prior to their death that could aid in determining the cause of death.
A report of death investigation is completed for every case involving a death. The report includes the name, age, race, gender, and residence of the deceased, as well we if any injury was reported prior to the death, and where the death took place. Reports of investigations also include information pertaining to the circumstances of death such as when and where the deceased was last seen alive, and pertinent medical and/or occupational history. Descriptions of the body including hair and eye color, tattoos, scars, height and weight, and signs of death or injury are also recorded. Lastly, a report of investigation will note if an autopsy was performed, if so, when, where, and by whom, and also if a cremation took place. Most importantly, a report of investigation will explain manner and cause of death. The following evidence will show two pictures taken of myself at the Office of the Chief Medical Examiner in Connecticut while attending several autopsies. I have also included a fictional report of investigation for the homicide of a 47 year old, Hispanic male who is the deceased victim of two stab wounds causing severe blood loss.
